Answers to questions about Immunization Forms

Which is the proper immunization form to use in my child care setting?

  • DOH 348-001 WA State Lifetime Immunization Record
    or
  • DOH 348-013 Certificate of Immunization Status?
    Are either of them OK for providers to use?

 ANSWER: CHANGE IN PRACTICE:

Only the "Certificate of Immunization Status"  form is acceptable for use by licensed providers.  The "WA State Lifetime Immunization Record" is meant for use by parents.

The form does not need to be the green, hardcopy record.  A photocopy or printed version is acceptable.  The Department of Health (DOH) informs us that they will phase out printing the green hard copy forms soon, and that the electronic versions will be all that are available.

Licensors and providers must check to be sure that both sides of the form are copied/printed, since the portion for parents to sign is on the second page of the form.

This represents a change in practice from an earlier policy clarification.  Therefore licensors must offer reasonable time to providers to come into compliance with this requirement when the "WA State Lifetime Immunization Record" is discovered in a child's file.

Varicella (Chicken Pox) Immunization Required

Beginning July 1, 2006, children from age 19 months up to kindergarten need to be vaccinated, have documented history of disease of a positive serology or be exempted from the immunization. Children entering kindergarten and sixth grade for the 2006-07 school year must prove their immunity to varicella. The Department of Health encourages verifying varicella immunity status for all children in grades Kindergarten to sixth.

National Resources

  • Associate Center: An Associate Center (AC) of the National Resource Center for Health and Safety in Child Care (NRC) is a regional network of agencies and organizations dedicated to enhancing the health and safety of young children in out-of-home child care within their state. For federal Region X, the Network has been designated as the AC. The Network was chosen, in part, because of our strong collaborative partnerships in early childhood/youth care and the public health arena. The AC will operate in conjunction with Healthy Child Care Washington and has adopted Healthy Child Care Washington as it’s name. Together, we will strive for local, state, regional and national scope.
  • The AC network will become the center of advocacy in making significant system change in child care delivery. In addition, the AC will continue to develop future leadership in the areas of child care health in Washington and the other states within Region X. Leadership development in child care health will empower child care providers and their community partners to better identify needs of children in their care. Local partnerships between child care and public health is essential to the provision of quality care and services for child care providers and the children they serve. With local leadership and partnerships, stronger systems for supporting families are assured.
